SNAPPS questionnaire is a self-report questionnaire that can discriminate between those with patellofemoral pain, those without patellofemoral pain, and those with other soft tissue injuries. The questionnaire has 13 questions and asks questions on clinical features, pain on activity, and pain on location.

Below are the paper and digital versions of the SNAPPS questionnaire. eSNAPPS file is in a JSON format and can be exported into survey platforms such as Online Surveys (previously known as Bristol online surveys). Guidelines to score results obtained from the eSNAPPS are here.  An Rmarkdown script that automates this process is also provided below. This can be opened using R and Rstudio, both are open-source.
